Accountability is naming who owns the result - and what happens when it slips.

Level 1 - Recognize

Accountability is clear ownership - one person answers for the outcome, the miss, and the fix.

Level 2 - Explain

Like one captain carrying the cleanup bucket - shared involvement without a named owner lets work float.

Level 3 - Use

Before the meeting ends, name one owner, what happens if it slips, and who reports back.

Level 4 - Connect

One owner prevents shared vagueness, but ownership needs enough authority to act and a blameless review when the system helped cause the miss.

Level 5 - Create

Build an ownership agreement that names the result, owner, authority, slip response, and review loop before work starts.
