Accountability = clear ownership — who answers for the outcome, the miss, and the fix.
Example
After a patient mix-up, our charge nurse named who was running the code cart and who was documenting — not “everyone was involved.” Accountability was one owner, one timeline, and one honest chart note before the shift ended.
